Here’s how it works:
﹀ Step 1 ﹀
Initial Contact with Homeless Community
Our team of trained personnel will travel to areas within the Greater Memphis Metropolitan Area to engage with the homeless community. Those who show interest in our program and services, and are in our demographic, will be escorted, using Big Dreams & Bigger Hearts-owned transportation, to our intake facility.
Those who are not interested will be given information regarding getting health insurance, temporary housing, and food banks.
﹀ Step 2 ﹀
Determine If Candidate Meets Initial Program Criteria
Once at our intake facility, our potential candidates will go through comprehensive medical examinations covering both physical ailments such as disabilities and illnesses, and mental health conditions including, but not limited to, depression, anxiety, drug addiction, and PTSD. After identifying the initial status of a potential admittee, those who are viable as possible program admittees move to proceed through the remaining steps of the intake process.
Those who are not viable as possible program admittees are given information regarding health insurance, temporary housing, and food banks.
﹀ Step 3 ﹀
Intake Process to Determine Program Admittees
Potential program admitters continue through intake and evaluation activities that will help us later design their Personalized Development Plan (PDP). These activities include, but are not limited to, therapy sessions, follow-up medical appointments, and disability accommodations. We aim to provide holistic care by addressing all aspects of health, ensuring our patients receive thorough and personalized treatment plans tailored to their individual needs.
Those who are determined to not be eligible for the program are given information regarding health insurance, temporary housing, and food banks.
﹀ Step 4 ﹀
Admission into the Program
Once admitted into the program, we offer a wide array of vital services tailored to support individuals in various facets of life. From providing stable housing solutions and facilitating transportation needs to offering mental health therapy, access to psychiatrists, therapists, and supportive communities, alongside valuable life coaching and comprehensive job training opportunities, all in alignment with their PDP. Our holistic approach aims to empower individuals toward personal growth, resilience, and fulfillment in all aspects of their lives.
By doing this we plan to build them up by starting at their roots so they can create their future on a solid foundation.
﹀ Step 5 ﹀
Admittee Graduates Program
A requirement to graduate from the program includes attaining gainful employment, acquiring life skills (e.g. financial literacy), and a permanent housing assignment. The program ends with a graduation ceremony to commemorate the participant’s journey to re-entering society.
The formal ceremony will include boasts of the candidates’ job placement, permanent housing allocation, and future goals!
﹀ Step 6 ﹀
Admittee Enters Forever Family Program
Our services do not end when a program participant graduates! We reach out at specific intervals to check in and encourage our graduates to reach out to us if needed. We will continue to provide a broad spectrum of support services aimed at empowering individuals to continue building upon their bright future.
Our Forever Family program will offer all program services from job placement and rehabilitation services to access to healthcare, and assistance with medication purchases, in addition to financial advisement and courses if the admitter once again finds themselves in need and/or struggling again. Our comprehensive offerings ensure holistic support for every step of the journey towards self-sufficiency and well-being as we believe a good support system can make a big difference.
